How come the changelog is incomplete?

Why is the changelog incomplete? And why is this often the case?

PDF refreshment issue. Thanks anyway.

Very strange browser behaviour that files don't update when a new file is introduced. Why would anyone wan't to look at an old file?

Maybe this is just a Chrome issue. Perhaps time to move back to Firefox.
